The Elder Care Support program is designed to deliver Aboriginal and Torres Strait Islander Aged Care support, connection and coordination to our Elders in our Community.  National Aboriginal Community Controlled Health Organisation (NACCHO) will work with Galambila to ensure our older Aboriginal and Torres Strait Islander people and their families are supported to understand, navigate and access aged care services they are entitled to.

The program aims to:

  • Support older Aboriginal and Torres Strait Islander people to understand and engage with the aged care system, to receive greater local support.
  • Reduce barriers across the aged care journey to increase the number of Aboriginal and Torres Strait Islander people accessing aged care services, achieving or exceeding equality with non-Indigenous people at a whole of system and program level.
  • Increase the number of Aboriginal and Torres Strait Islander people receiving care on Gumbaynggirr Country.

Elderly services:

  • Health assessments
  • Geriatrician clinic
  • Referral pathways
  • GP Chronic Care Management Plans (GPCCMP)
  • Advocacy
